Yes. You get one primary caregiver and one backup — not a different person each week. That consistency matters, especially for someone who does better with a familiar face.

Skilled nursing is care that legally must be provided by a licensed nurse (RN or LPN) — wound care, injections, monitoring complex medical conditions. Regular home care is non-medical daily living support.

If your doctor says your loved one needs nursing-level care at home, that’s what we provide.

 

Our office team is available during standard business hours. Our 24/7 care coordination line is there for anything urgent — calls, questions, emergencies.

Yes. Our dementia-trained caregivers know how to recognize and respond to confusion and agitation, use communication techniques that work with memory loss, and keep routines calm and predictable. It’s training that goes beyond general personal care.

You’ll need to complete an application, pass a home study, clear background checks, and finish state-approved training (called IMPACT training). It sounds like a lot, but it’s designed to protect children, and our team walks you through every step so nothing feels mysterious. We stay involved after placement too — not just during the approval process.

Personal care is non-medical, hands-on help with everyday activities: bathing, dressing, grooming, meals, mobility, medication reminders. Home health is when a licensed nurse provides medical treatment — wound care, physical therapy, clinical monitoring — usually ordered by a doctor.

We provide personal care directly. If your loved one needs nursing-level care, that’s our skilled nursing service.

Yes — this is exactly what Diamond was built for. A young adult aging out of foster care who also needs disability support doesn’t have to start over with a new provider. History, documentation, and standard of care move with them.
